CAN I SEE YOU VIRTUALLY?
Yes! All appointments are currently virtual until further notice.
WHAT ARE YOUR SPECIALTIES?
I specialize in eating disorders, disordered eating, female athlete triad, chronic dieting, intuitive eating, mindful eating, hypothalamic amenorrhea, and mood & anxiety disorders. I use a Health at Every Size® approach. Visit the ABOUT page to learn more.
DO YOU TAKE INSURANCE?
Client Centered Nutrition is contracted with Regence, Premera, Blue Cross Blue Shield, Lifewise, First Choice Health (preferred provider), and Kaiser Permanente PPO plans. If Client Centered Nutrition is out of network with your insurance provider, a superbill can be provided at your request for you to seek possible reimbursement from insurance. Please note that a superbill does not guarantee insurance reimbursement.
I recommend contacting your insurance directly to verify coverage before your first appointment. For your convenience, I've put together a list of helpful questions to ask insurance companies regarding reimbursement. QUESTIONS FOR INSURANCE:
Does my insurance plan cover outpatient nutrition counseling? Medical nutrition therapy? Preventative nutrition counseling? Dietary counseling and surveillance?
Is there an out-of-pocket max or a deductible I must first meet?
Must I get a referral from my primary care provider (PCP)?
Are there specific diagnosis code restrictions?
How many sessions (or units) am I eligible for?
WHAT FORMS OF PAYMENT DO YOU ACCEPT?
Forms of payment accepted include: cash, check, credit cards, debit cards, and HSA/FSA accounts. You are responsible for payment in full before or at the start of your appointment. Please make checks payable to "Client Centered Nutrition, PLLC".
DO YOU OFFER A SLIDING SCALE?
Yes, there are a limited number of sliding scale spots available. Please contact me (Amanda@ClientCenteredNutrition.com) if you are interested in learning more.
WHAT IS YOUR CANCELLATION POLICY?
If you cannot attend a scheduled session, please call to cancel or reschedule. There will be no fee if a phone message or an email is received 24 hours in advance of the scheduled appointment time. If you miss or cancel with less than 24 hours of notice, then you will be charged for the full price of the appointment. If you are late for an appointment, the appointment will still end on time out of respect for the client scheduled after you.
WILL YOU WORK WITH MY OTHER PROVIDERS?
Yes! I believe coordinating care with your other providers, and past providers, is highly valuable. A release of information (ROI) must be signed before I can communicate with any outside providers. This will be addressed in our first session.
WHAT DO I NEED TO DO TO PREPARE FOR OUR FIRST APPOINTMENT?
Once you schedule an appointment you will receive an email with all the information you need. You will be provided a link that will direct you to our secured, HIPAA compliant platform. Complete the forms online and you are set!
